Veterans’ Diabetes Management Program

A fully funded* diabetes education and management program delivered by a Diabetes Educator.

Our fully funded* program supports you to better manage the life-threatening disease. It integrates early intervention, education and ongoing support and monitoring. This program is an add-on service available for participants of the Veterans’ Better Life Program.

How it works

We support you at every stage from pre-screening and education, right through to ongoing support and monitoring.

How the program works:

Early Intervention

  • We understand the importance of early intervention prior to serious symptoms developing.  That’s why we undertake pre-screening within our Veterans’ Better Life Program and provide you with a diabetes risk assessment to help you and your doctor determine next steps.

Education

  • We hold one-on-one and workshop sessions to help you understand the disease and how to manage it. Our approach balances the need to communicate the seriousness of the disease, while empowering you to take control of your health more.

Ongoing support and monitoring

  • We provide ongoing support and monitoring to help you achieve better health outcomes—we are your personal diabetes coach providing ongoing support to help you stay on track!
This includes:
  • Case management with scheduled check-ins, tracking, and advice
  • Facilitating motivation to avoid diabetes burnout
  • Working with you to set personally relevant and achievable goals that prioritise lifestyle changes
  • Developing individualised, adaptive diet and exercise plans to help you achieve your goals and better manage your health.

Open to Australian veterans diagnosed with diabetes

The program is available to participants of the Veterans’ Better Life Program with a DVA Gold Card that have been diagnosed with diabetes or prediabetes (or at high risk of developing diabetes).

We welcome DVA clients.

Fully funded for DVA Gold Card Holders

There are no out-of-pocket costs for the program. DVA Gold Cards are accepted as full payment upon a GP referral.

How is your program different?

Our program is a specialised veterans’ diabetes health care service delivered by a diabetes educator, with support from a dietitian and psychologist as needed. It’s delivered in a relaxed veteran friendly environment and integrates screening, education, and ongoing support and monitoring.

Who is eligible for the program?

The program is available to participants of the Veterans’ Better Life Program with a DVA Gold Card that have been diagnosed with diabetes or prediabetes (or at high risk of developing diabetes).

The program is available at health clubs on the:

How long does the program run for?

There is no set duration. The program is tailored to each veteran’s needs in consultation with their GP.

How can veterans access this program?

Eligible veterans just need a referral from their GP or hospital discharge planner, and we’ll do the rest.

What is the prevalence of diabetes among veterans?

Every day, someone in the veteran community is diagnosed with diabetes—while many more still go undiagnosed. Within our Veterans’ Better Life Program, AUSDRISK screening has identified that nearly 60% of screened participants either have diabetes or are at high risk of developing this life-threatening disease.

How many sessions does a referral cover?

A referral covers one 12-session treatment cycle. Veteran clients may have as many additional treatment cycles as their GP determines clinically necessary. Clients deemed a high risk may be eligible for a tailored referral and review arrangements if their health, treatment, or wellbeing would be adversely affected by the standard 12 session treatment cycle.

What is the Veterans’ Better Life Program?

It’s a fully funded* one-stop-shop with all treatments delivered in one place by a multidisciplinary allied health team (including a physiotherapist, exercise physiologist, and psychologist). The program is an evidence-based health care service that’s tailored to the veteran’s needs and delivered in a relaxed veteran-friendly environment.
